Step 1:
Obtain a copy of your most recent medical evaluation from your primary care provider. This office note should include a list of medications and any medical diagnosis that you have currently. Also bring with you a copy of any relevant diagnostic tests such as MRI or CT scans.
Step 2:
Make your appointment. Be here 30 minutes prior to your scheduled time to complete the necessary paperwork.
Step 3:
Bring everything with you for your scheduled time (or call to cancel).

Initial Consultation: Budget 30 minutes prior to your appointment to complete the mandatory paperwork. Also allow for up to 30 minutes for your appointment.

Follow up consultation: Budget 15 minutes prior to your appointment to complete the mandatory paperwork. Also allow for up to 15 to 30 minutes for your appointment

You will need to have all the forms completed prior to the state issuing you a medical marijuana license.
You will need to have a valid email address for all state mandatory communications to receive your medical marijuana license.
